Two people in Worcester, Massachusetts had one of the worst mix ups ever. A surgeon was scheduled to remove a patient’s kidney, which had a tumor on it. Instead, the doctor removed another patient’s healthy organ.

How’d the mix up happen? The two people have the same name. Authorities report the surgeon failed to check the patient’s birthday, which is proper identification protocol. As it turns out, the two people were several years apart in age.

St. Vincent Hospital says it is taking “all necessary steps” to prevent a similar error.
